School-Based Counseling Services for Portland Public Schools

Sankofa Counseling providers are available at various Portland Public Middle and High Schools for the 2025-26 school year. If you are a BIPOC or QTBIPOC student or parent seeking support, follow the steps below to connect with school-based counseling services.

 

Step 1: Confirm student is a fit for services using this form.

& Submit a Release of Information (ROI) between Sankofa and PPS using the link below.

Step 2: Find and submit your school’s referral form using the links below.

    • Once program eligibility is confirmed and ROI is complete, complete the Online Referral Form: Click your school's link above to access and fill out the referral form.

    • After a referral is received, a Sankofa counselor is tentatively assigned.

    • Your student’s School Counseling Team receives a checklist of required paperwork (insurance, consent forms, etc.) to collect from the student or legal guardian. *Please note, this process can take up to 2 weeks.

    • First Meeting: After referral is complete, the student meets with a Sankofa counselor for their intake session.

    • Consent Forms: Must be signed within 2 sessions to continue. Forms will be provided by the student’s Sankofa counselor and can be completed online, on a Sankofa laptop during session, or on paper (returned to school staff for secure upload).

      • Note: Students can be scheduled for future sessions only after all paperwork is complete.

    • Attendance: Students are expected to attend regularly. Services may close if there are 3 or more cancellations or missed sessions within 5 weeks.
